The crash killed all three crew members and 12 people on the ground and injured 23 others.<\/p>\n<p>The incident occurred at approximately 5:14 p.m. on November 4, 2025, when UPS Flight 2976, a McDonnell Douglas MD-11F cargo aircraft, crashed shortly after taking off from Runway 17R. The aircraft was bound for Honolulu, Hawaii, on a domestic cargo flight.<\/p>\n<p>Investigators determined that the aircraft&#8217;s left engine and pylon separated from the left wing during takeoff. The aircraft then crashed into an industrial area near the airport, where a large post-crash fire caused extensive damage.<\/p>\n<p>The National Transportation Safety Board (NTSB) and Federal Aviation Administration (FAA) launched a major investigation immediately after the accident. The NTSB later recovered and examined critical aircraft components, flight-recording data and maintenance information.<\/p>\n<p>Confirmed Victims and Crew<\/p>\n<p>UPS identified the three crew members aboard Flight 2976:<\/p>\n<p>Captain Richard Wartenberg, 57<br \/>\nFirst Officer Lee Truitt, 45<br \/>\nInternational Relief Officer Captain Dana Diamond, 62<\/p>\n<p>All three crew members died in the crash. UPS publicly released their names on November 6, 2025.<\/p>\n<p>Eleven people on the ground were initially confirmed killed. Another seriously injured person died 51 days after the accident, bringing the total death toll to 15. Twenty-three people on the ground were injured.<\/p>\n<p>The crash affected businesses and workers in the industrial area immediately south of the airport, including facilities near the aircraft&#8217;s impact site.<\/p>\n<p>What Happened During Takeoff<\/p>\n<p>According to the NTSB investigation, Flight 2976 began its takeoff from Louisville shortly after 5:14 p.m.<\/p>\n<p>Airport surveillance footage showed the aircraft&#8217;s left engine and its pylon separating from the left wing during the takeoff sequence. The aircraft continued forward but was unable to remain airborne and ultimately struck the ground.<\/p>\n<p>The crash produced a massive fire fueled by the aircraft&#8217;s jet fuel and destroyed the airplane. Emergency crews responded rapidly as firefighters worked to contain the flames and search the surrounding area.<\/p>\n<p>The NTSB later recovered the aircraft&#8217;s cockpit voice recorder and flight-data recorder as part of the investigation.<\/p>\n<p>Investigation Finds Evidence of Fatigue Cracking<\/p>\n<p>Investigators have since focused heavily on the structure connecting the engine to the aircraft&#8217;s wing.<\/p>\n<p>An NTSB examination found fatigue cracking in the spherical bearing assembly associated with the engine-pylon structure. Investigators reported that the cracking extended through the bearing race and covered approximately 75% of the fracture surface, with the remaining portion consistent with an overload failure.<\/p>\n<p>The discovery has raised questions about the design, inspection and maintenance history of the MD-11&#8217;s pylon attachment structure.<\/p>\n<p>Investigators also examined previous reports involving similar components and the inspection intervals used for the aircraft structure.<\/p>\n<p>NTSB Investigative Hearing<\/p>\n<p>In May 2026, the NTSB held a two-day public investigative hearing in Washington, D.C., examining the circumstances surrounding the crash.<\/p>\n<p>The hearing examined the design of the MD-11 pylon, structural requirements, inspection procedures, previous failures, maintenance information and FAA oversight.<\/p>\n<p>Investigators also examined whether information about previous bearing and structural failures had been adequately considered when inspection requirements were established.<\/p>\n<p>However, the NTSB has not yet issued a final determination of probable cause. The agency emphasized that the hearing was part of the fact-finding process and that additional analysis is required.<\/p>\n<p>Questions About Maintenance and Safety<\/p>\n<p>The investigation has raised important questions about how the MD-11&#8217;s engine-pylon structure was inspected and how previous reports of component problems were handled.<\/p>\n<p>NTSB documents show that investigators found fatigue cracking around the circumference of the bearing&#8217;s designed recess groove. The evidence is now being analyzed alongside maintenance records, engineering data and information about the aircraft&#8217;s operating history.<\/p>\n<p>Boeing, the aircraft&#8217;s type certificate holder, has also submitted technical information to the NTSB. Boeing has argued that the MD-11 pylon structure was designed as damage-tolerant and that its certification and maintenance requirements were intended to address structural failures.<\/p>\n<p>The investigation is therefore examining not only what physically failed, but also whether existing design and inspection practices were adequate to detect the developing problem.<\/p>\n<p>Investigation Remains Open<\/p>\n<p>NTSB Chairwoman Jennifer Homendy said following the May 2026 hearing that investigators were moving into the analysis phase, where they would evaluate the evidence, develop recommendations and prepare the final report.<\/p>\n<p>The NTSB said it was working to complete the investigation within approximately 12 to 18 months of the accident, potentially bringing a final report by May 2027.<\/p>\n<p>For now, investigators have established a critical part of the accident sequence: the left engine and pylon separated from the wing shortly after takeoff, followed by the aircraft&#8217;s loss of control and impact.<\/p>\n<p>But the final explanation of why the structural failure occurred \u2014 and whether design, maintenance, inspection or other factors contributed \u2014 remains under investigation.<\/p>\n<p>The tragedy continues to be remembered in Louisville, where the crash affected UPS employees, nearby businesses, emergency responders and families across the community.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Louisville, Kentucky \u2014 Federal investigators continue to examine the cause of the deadly UPS cargo plane crash at Louisville Muhammad Ali International Airport, nearly 10 months after the accident.
